Initial Notification: At 9:58 PM (local time) on 14 July 2009, the regional NOAA Scientific Support Coordinator (SSC) was contacted by USCG Sector New Orleans relative to a Hydrofluoric Acid incident at the Chalmette Refinery. The cause of the incident was an equipment failure. Apparently, acid used in the refinery process that was entering into a heat exchanger and associated cooling water. The effluent water was being discharged into the Mississippi River.
